Annual Athletic Pre-Participation Medical Paperwork This paperwork MUST be on file before beginning practice for any sport. If you have questions, please contact Mr. Basye at jbasye@concordia.k12.mo.us or 660-463-2246.

Please download the document and fill it out using adobe reader (Can download from https://get.adobe.com/reader/ , uncheck both boxes that are under the optional offers and then select download acrobat reader and install)unless you are using a different pdf software not the pro trial button.
To create a digital ID for esigning the documents Instructions:
Click in the signature field box
Select configure digital ID
Select Create a new Digital ID
Select save to windows certificate store
Enter Name, Enter email Country/Region: United States, Key algorithm:2048-bit RSA,
Use Digital ID: Digital Signatures
If it ask for a password then type in a password then hit continue
Then click the sign button
At that point it is going to ask you to save the document, save it to a location of your choice and then send to jbasye@concordia.k12.mo.us for the High School Students
